Is it only my watchface (Utility) or is your AW5 also only updating the red notification icon when the watch is activated? I find this quite irritating as the watch face is updated every second and there should be no problem updating the red icon also?
Am I missing something?
 
Mine can take some 2 to 5 seconds between the watch vibrating and the red dot showing. This is on AOD, with raise to wake off and without waking up the watch. Did you try a reset?

This is one of the reasons I do not use AOD, along with the extra step needed with waking the AW5 to see what the notification is.
 
Actually I like AOD a lot. Coming from a Garmin watch I am used to have AOD. But it is sometimes confusing and not consistent.

I really like AOD but why on earth didn’t Apple give us an option to wake screen on notification. My 3 year old Samsung watch had it.

Yes, would be a good thing. Still do not understand when a notification is being displayed all over the screen and when there is only the red dot and when there is nothing. Got some research to do.

I find it to be a bit inconsistent as well. Some email notifications open up unrequested, while the other 98% of them just give a vibration and a red dot on the AOD.
I have next to no experience of the various iPads, but I'm sure WatchOS still is the most unorganised and inconsistent system Apple offers. I guess it's pretty hard to develop a perfect UX on such a small display.

It is very consistent. When you get a notification, there is a sound/vibration. If you wake the watch immediately (by raising or tap), the notification will be displayed. If you don‘t, the red dot will appear instead.

That is how I thought it would work, but sometimes it just lights up and shows a notification. I have no raise to wake.

How strange, I never saw this happen on my Watches. OTOH, I have never disabled raise to wake. Maybe with raise to wake disabled, the Watch discriminates between different notifications?
