Recent upgrade,
from iPhone 3Gs to 5s..........:) (wife's old phone, I get the old one! :)
Phone holds charge quite well but drops rapidly during a long call, is this normal,
or should I have this old battery replaced

Its normal with 5years old batteries:)

What does battery settings say about battery health?

If it was never replaced than its time as they are designed just for 500 cycles

No point having the battery replaced. It will cost at least USD 79. Might as well buy an SE.

Will put a new phone on the Xmas list

Well you can try replacing your own battery first before getting a new phone. I have a 5S too and I replaced mine using a Scanditech replacement battery on Amazon. I just followed their guides and well, my 5s is now good as new.

It's inexpensive too.
