When you get Apple to replace an iPhone's battery (in this case, outside of Applecare coverage), do they actually replace the battery in the phone you send, or do they just send you a refurb? I sent in an Apple Watch for a battery replacement and they sent me a new one and I wasn't sure if it was the same with the phones.

No, you will receive the same phone back as long as nothing goes wrong during the repair. If something goes wrong with the repair, they will give you a refurbished device. Watches and iPads are a different story. When you send one of those in for a battery replacement they only send you a refurbished device..
