Assistive Touch and Hand Gestures are off. It doesn’t always show, just sometimes.
 
Turn Visual Signal off:

Settings>Accessibility>Assistive Touch>Hand Gestures>Activation Gesture>Visual Signal.
 
Try a reboot, hold both buttons until the Apple logo appears. (Not action button if you have Ultra).
