I've had this problem since at least iOS 6. I have used Plantronics Bluetooth headsets and at random the volume resets itself to the lowest possible volume and I have to go on the headset and raise it to the highest it can go. Recently I've found that it seems to reset as I get into the phone app and or when the phone locks and a noise is activated. I have gone through multiple Plantronic headsets and they all do them. I'm using a Voyager Legend right now.
My only other weird variable is that I use multiple notification sounds for different e-mail accounts and there is a weird bug with that where random sounds occur that shouldn't be. Not sure if it's related but wanted to reach out and see if anyone else besides me encounter this problem.
