Hello

I play music from spotify via bluetooth to my car stereo. I am thinking of buying a bluetooth headset for phone calls in the car. Will I be able to play to spotify via the stereo but accept calls via the headset? My stereo also does phone calls but the mic quality is poor.
 
Probably not exactly like that, as the currently linked bluetooth device will take the call, but you can switch to the headset using the phone (and maybe using your car interface, depending on how smart it is. Newer Honda's can switch to the phone, although I haven't tried it with a second bluetooth device.)
 
I do have a bluetooth dongle attached to my iPod connector (Auris), and use the bluetooth on the radio for calls, you will be ok. If I'm not mistaken, that is one of the benefits of bluetooth 4.0...

i can indeed connect iphone 5 to my bluetooth headset and bluetooth stereo at the same time.

The music plays automatically via the stereo and calls come in to both simultaneously. Whichever device i answer the call from is automatically set as the telephone audio
 
Easiest way to find out is if you can borrow a headset and find out.

The phone can do it. It's whether the head unit or hands free can.
