Just got my new 30gb video ipod, and one of the headphones doesnt work. My friend says theres a form I can fill in on the apple website to order some more, anybody know the URL for this as I cant seem to find it?

Thats if it exists...
 
I would call AppleCare and have them replace them. I don't know about the form, but that route would probably cost $15 or so as opposed to calling them and letting them know they're broken.

If your iPod is still under warranty, there should be no charge, but they will give you a mailer to send the old earbuds back. You will probably be asked for credit card information, but won't be billed (and they will warn you ahead of time and give you the opportunity to back out of there would be a charge).
