Are BTO Macs just pre built stock models re-opened and configured per customer requirements?

Or are they actually built from scratch and configured per customer requirements?
 
My guess is the former. I doubt that Apple receives your order and then sends a special request to China.

I would suggest, however, that at product launch Apple has some upgraded models pre-built they know will sell, just to help deal with the initial launch. This is purely speculation on my part, BTW.
 
Usually the apple stores stock a few variants, like Ram and Hdd size, and of course glossy and matte, but real custom things will be hand done.
 
I dont think that they will disassemble and assemble again a brand new computer just to put the items you requested customized. I think that they alredy receive them as it is, They know very well the demand of each configuration, so they already come from the factory that way. At most they may change the memory config, that is easy to do.
