Hi looking for advice

Want to buy an iPhone 5s when I'm in the US later this month.
Want to get device only in an apple store and use it with an O2 sim when I get back to Ireland. (haven't got the sim yet)

Are there any issues or is it a complete non starter

No issue as long as you purchase it directly from Apple (Apple store).
 
Just keep in mind you wont have warranty in Europe if you purchase an american iPhone, at least during the first year (as has happened with the iPhone 5).

There are stories of people getting an american iPhone replaced in Europe, but that is not the norm and the official word from Apple is that warranties on iPhones are regional.
