The feature that is absolutely mandatory, as in no app without this feature is of ANY USE WHATSOEVER to me:
If an alarm is set to go off at a given time, it goes off, and then does not cease going off until interacted with. I don't care whether it's 5 seconds or 5 hours. I want the device to constantly make sound until I acknowledge the event.
I am sometimes not in the room with an iPad for a couple of minutes. I don't always check them obsessively when I'm busy focusing on other tasks. I need an alarm to continue until I respond to it, so I don't have the current situation, where I miss the brief interval of beeping and the ipad then sits there silently, not mentioning that there is a pending alert I haven't responded to.
Palm and Blackberry had this, I've found nothing that can do it for iOS yet. (Or for Android, though I haven't looked there so much.)
iCal is being really disappointing to me; it doesn't handle things like recurring events that are three days a week, and I have not yet found a way to make it stop copying appointments to other devices. But mostly, the alarms are useless.
