Call forwarding menu bar "ON" for no reason.

I just got a replacement iphone for a previously defective one. When I inserting the sim card in the phone the little icon for call forwarding came on in the menu bar. I thought it was on and went since its a feature I never used. When going into the phone settings I realized the call forwarding option was not turned on. What do this icon mean then? Am I experiencing a software bug?

My iPhone is a 4S jailbroken on 5.1.1 But it was not jailbroken when I first experienced this specific issue.
