Can some help me with the name of this APP?

I dont know how and where I downloaded it from. It has extended system preferences with an option of 5 app docks and custom carrier name. I used to launch it from the setting>system preference

I think you're referring to Supreme Preferences. It can be downloaded from the ModMyi repo I think.
