Hey,

I’ve a Samsung QLED. Our internet is terrible.

If I rent or buy a movie is it possible to download for offline viewing in the Apple TV app or would I need an actual Apple TV box for downloads?
 
You can’t download videos on an tv. I highly doubt you can do it on the tv app on a smart tv.
 
It doesn’t have a hard drive. The flash storage is for apps and their data. The original tv was the only tv that could have media stored on it. The current tv is a streaming device.
