Can't format USB thumb drive under OS 9

I'm trying to format a USB thumb drive (Sandisk Cruzer) under OS 9 on my eMac, but it keeps saying formatting failed. I've tried formatting under all three options - Mac OS Standard, Extended and DOS -- but no dice. Any clue as to what's going on?

EDIT: The thumb drive is 4GB.

Did you format it HFS+ and install OS9 Drivers?

I did not do the latter, but then again there was no such option. The moment I plug in the USB, The format dialog box appears, and regardless of filesystem choice, the format fails.

I borrowed a copy of Tiger and ran its Disk Utility and did as you said, and now, when I plug the USB in, I get a warning saying something like "The disk has a problem. Some of the files may be lost. Please check to see if any recent files were lost and use a disk restore program" (Sorry this isn't the exact wording, my copy of OS 9 is in Korean so I translated the message. I ignored the message and tried copying a few files...no dice.
