Can't see 2nd monitor after NVIDIA driver update

I have an 8800GT in the Mac Pro and after the latest NVIDIA driver update on Vista 64 Pro (I thought I should do it as it had been a while) try as I might (and I have tried a lot) the 2nd monitor is only visible when it (the 2nd monitor) briefly shows the 'BIOS' screen on startup (for some weird reason, it's always done that) but once windows has loaded, no 2nd monitor.

I have tried force detect and all the usual stuff like swapping the leads round etc.

Any ideas?
