I just got a Verizon 4G LTE iPad and I'm having some trouble with the hotspot feature.
I can connect to the hotspot without any problems using my iPhone 4S on AT&T.
However, about 5-10 minutes after I lock the iPad screen, the hotspot connection disappears on my iPhone and goes back to AT&T. If I look in the WiFi settings on the iPhone, it does not show the iPad SSID.
When I unlock the iPad and go into settings, hotspot shows "on" but wireless says "not connected". If I select WiFi, it will change to "on" and then the hotspot is working again.

I called Apple and their only suggestion was to turn off auto screen lock which did nothing. I also went to a verizon store, but couldn't replicate the issue there. It stayed connected for about 10 minutes so I left the store and 2 miles down the road I looked at my phone and it had disconnected.

Any ideas? I guess I could try leaving the iPad screen on, although that would drain my battery pretty quick.
One more thing - I'm not leaving the range of the hotspot, I've been within 3 feet of it on all my testing.
 
The wifi hotspot will always drop the connection after sleep, and not always pick it up. If you want to stay connected, tether over bluetooth. The speeds are slightly slower but it uses less power and will stay connected.
