Capture card / TV Tuner

Hello, I'm a huge newbie here so please bear with me...

I am looking for a piece of hardware / software which would allow me to record TV shows and Playstation gaming sessions.

Now, is it possible to "capture" the image form a TV ? I guess if the TV has a video OUT then it is.... or do I need a dedicated TV tuner if I want to record TV shows?

Which leads to my next query. Any way to "capture" from a HDMI equipped device?
