CDMA vs GSM

It looks like Apple's website no longer offers a SIM-free version. Apple's website suggests that if you choose Verizon or Sprint then it will work with every GSM and CDMA carrier, but if you choose GSM it will only work with GSM. So... why do they even bother offering just a GSM one instead of selling the GSM/CDMA one to everybody?

Also, a lot of these companies try to charge upgrade fees which is ridiculous because I just want to swap out my SIM card myself. Apple's website makes me choose a carrier and says they work with my carrier... So do I have to give Apple my phone number and do they let T-Mobile know I'm getting a new phone and so T-Mobile can try to charge me?
