Change mac res to 3840X2160

I just got a Samsung 4K monitor to pair with my mid 2011 iMac and I can not find the correct values to plug into the SwitchEesX custom resolution pane to get my Mac to accept the new res and make it active. Also I am switching from a Thunderbolt to HDMI to display port to HDMI cable. SO...What is the solution? I am going crazy here!

Why all the adapting?

Ideally you would use TB to DisplayPort.

Hdmi isn't going to give good results.
