CMD-tab doesn't always bring apps to front

I've had this problem for a while, and I'm sort of sick of it. Hoping that someone will know of a fix. Often, when I use CMD-tab to switch from an app on desktop "A" to an app on desktop "B", the app on desktop "B" doesn't come to the front. It seems to happen a lot with Thunderbird and Terminal, but it happens with other apps too. Sometimes restarting the computer will make it stop, but often it doesn't help. Anyone know of a fix?
