Disclaimer: I was forced to add a tag to the title but have only seen this issue on my cMP5,1. As such, I have no idea if this exists on earlier MacPros.
At the very start of the boot process, both operations find the same root device. However, only during power-on are the following messages re: finding a boot device seen in the boot-screen; again, these messages are not seen during reboot:
That said, my research points to USB mapping being the culprit but here's the thing; all USB ports appear to work without having done the mapping. In addition, all of the guides I have seen for USB mapping presume it is being done on a Hackintosh, not a real Mac, so I'm hesitant to trust those procedures and solutions on this platform.

- Platform: cMP5,1 (2012) --- 2x 5645 --- 64GB RAM
- GPU: Radeon Pulse RX 580 8GB
- OC version: 0.8.0; plain vanilla Clover install except for the following (all done for troubleshooting purposes):
- ADD "-v" to the nvram boot-args
- ADD

kext - CHANGE
config.plist
: Kernel -> Quirks -> XhciPortLimit = True
- OS version: High Sierra (10.13.6)
- USB peripheral: Inatek KT-4004 (Fresco FS-1100 chipset)
- Verbose boot is enabled
Issue
The system will boot from power-on with no issues but hangs when trying to reboot. The final boot-screen message is "Waiting for root device" accompanied by garbled text and the "prohibited sign". kext and all ports appear to be mapped correctly. However, one thing that concerns me is the output from the ioreg
command. The Dortania guide says that if any output is seen with that command, then port\controller renaming (not mapping) is required but it is not clear to me how that would be done on a Mac. So all this coupled with several posts by others who say they have this exact same setup --- including the Inatek card --- working with no mods, have compelled me to reach out to the community.Anyone have any ideas, thoughts, suggestions?
