I have a mini-displayport - dvi adapter which I use to connect my macbook to my external screen (Del 19") and roommates TV (Samsung big flatscreen thing via a dvi - hdmi cable).

It is always a pain to get it to connect. When I plug it in when the macbook is running, the screen fades to blue for a couple of seconds as if it is doing something with the video card and then comes back on with nothing on the external screen.

It seems I have to restart the macbook with the cable connected to get it to register the external screen and even then I'd say the success rate is only around 50%. Is this a common issue (or how it should be working?).
