Constant Powerbook HD Access

I don't have a clue what's going on here, for three days now my HD has been in a state of constant disk access - mp3 playback is awful, importing to iTunes can cause the HD to lock completely, and in general the system randomly hangs for periods of 30 seconds or so. Help!

i know this is prolly obvious but back up immediately! and open disk utility and check the SMART status of the drive.

Uh oh. My PB is now stuck on a white screen starting up.
