Contact app with more intuitive notes and follow-ups

Can anyone advise me please?

I'm looking for a contacts app that will allow me to store date-stamped notes against a contact, schedule a follow-up reminder (ideally in the Reminders or Calendar apps) and alert me on that day?

Check out Daylite by Marketcircle. It's probably more than what you are looking for, and a little pricey since it is way more than a Contact app. But, I use it and love it. I have it on my Macbook Air, Mac Mini Server, iPad, and iPhone and they all keep in sync automatically.

I usually ask for features to be added and Raj has always tried his best to add my requests. I know they are finishing up the iCloud scenario for the iPhone version to hook up with the mac version.
