Used to have this problem with my 3GS, now it's back with my iPhone 4...
Basically if i add a new contact via the phone app (keypad, or resents list) it does not get added to my contacts, it only gets added to the 'All on my iPhone' group. I use Google sync (exchange) and it's not added to my Google contacts 'group'.
However, If i go into Contacts, press the + button and add a new contact it gets added to me Google contacts.
I cant figure out why one method adds to to my phone and one adds it to my Google contacts (where i want it). Drives me mad because sometimes i think im missing a contact only to find i need to switch groups. There is no way to move it that i can see.
The weird thing is, for a while this never happened. From a fresh install, i only had a group called Google, but they 'All On My iPhone' appears - which some contacts end up in sometimes. A reset on my 3GS fixed it, but i did come back.
