The Magic Mouse doesn't have a middle button and the keyboard cmd-c/-v doesn't do it, so how does one copy and paste in xterm in XQuartz?

Shift-insert (keyboard) helps. Depending on how you're configured, you may want to have xterm set to use its "selectToClipboard" resource (XQuartz select/paste is kind of fragile for the primary selection).
