What would be the cost, im weighing up AppleCare for peace of mind. Only got a few days left (a year goes fast these days!)

so about £150 (UK) same for just fixing the phone?

Check your local apple support site. What you want is called an "out of warranty repair". Price will vary a little by country as well as iPhone model.

Wirelessly posted (Mozilla/5.0 (iPhone; U; CPU iPhone OS 4_3_3 like Mac OS X; en-us) AppleWebKit/533.17.9 (KHTML, like Gecko) Version/5.0.2 Mobile/8J2 Safari/6533.18.5)

They don't fix it unless it's the back glass. They swap it take your money and give you a 90 day warranty. If it's the back glass it's £25

How damaged is it O.P?

If you approach them nicely and you tell them the truth of what happened to your phone, they might give you a free replacement. ;)

That's from hearing stories about Apple's wonderful service though...

Its not damaged yet... I am almost out of warranty and as I have been very happy in the past with Apples replacements (in warranty) I was thinking about AppleCare

If it's not of significant damage when your warranty expires, you might be able to get a free-of-charge replacement if your Genius is kinda. But as mentioned the out-of-warranty service costs $199 + tax compared to Apple Care for $69.
