Hey guys, I tried to search for this via google but couldn't find too much specifically for whats written. It's beyond what I can interpret so hoping someone here can tell me why my MBP keeps crashing. The only thing I've noticed is that it's usually when waking up from sleep mode.... any ideas? I'm hoping its not on its last legs...!

From that log, it appears to be a GPU problem and that is not something you will be able to fix yourself.
 
What model is it? If Weaselboy is correct (and there's no reason to doubt him), the MBP may be within apple's repair program for MBPs that have defective GPUs

Ackk... yeah those were part of a recall for the GPU like Mike mentioned, but according to this link the recall period has ended. I believe they were covering them for three years from the date of purchase. Might be worth a call to Apple to plead your case though.
