Creating EBook Style PDFs - Chapters ect.

I am curious if anyone knows how to create an "ebook" style PDF on the Mac 10.6 Snow Leopard - Hopefully using Preview.

See the attached for an example - basically on the sidebar - be able to jump to the different chapters and whatnot.

Thanks - I am in the process of getting my textbooks converted over and wanted to contain it all in a single file
