Custom icons broken in iOS 12?

I searched but didn't come up with any hits, so my apologies if this has been asked already...are custom icons working for anyone?

I tried to change the Carrot weather app icon and the NHL app icon and both stay at their default. Not the biggest of problems but I'm curious if I am doing something wrong on my end?
