Custom Profiles and Exceptions on Iphone similar to Blackberry?

I got the new Iphone4 and I love it.

Coming from a Blackberry I really miss the Custom Profiles and Exceptions. Specifically how I was able to have my phone on silent EXCEPT if certain people called or texted. This was highly customizable.

Is there an App that can give me the ability to do all of this?
