macOS Custom Titlebar Obj-C + Cocoa?

I am trying to make a custom title bar for my program. How can I achieve this?
Step by step replies are much appreciated! Here is a picture of what i'm talking about:

Start with an unstyled window, then render the new elements yourself.

You are meant to take it, work out how it works and then write your own code. Do not expect to find exact code that does precisely what you want that you can copy and paste (or basically steal).
