i am running 4.1 and had 4.1 saved by cydia previously.. today i went into my cydia and said i also had 4.2.1+ saved.. if im running 4.1, how did that 4.2.1 come up and why does it say 4.2.1+ ?

My cydia has done the same, so does that mean if I can get hold of the iOS 4.2.1 b3 file, I can get an untethered jailbreak? (I've stayed on 4.0 for now until I'm sure its ok to upgrade).

Technically yes, but it's not for the faint of heart. Based on what you didn't understand of the SHSH save process, I'd suggest waiting for a more stable option.

Cydia only need your phone ID to request your SHSH to Apple servers, it have nothing to do with what firmware you are running, but instead with what firmware is Apple currently signing.
The "+" is because the 3GS had two releases of 4.2.1 (4.2.1 & 4.2.1a) those firmwares generate 2 different sets of SHSHs and Cydia is saving both.
