I have a couple web pages I use for work that I created a bookmark to save to the Home Screen on an iPhone and iPad.
Can’t figure out how to delete them!
Hold the icon does not give the option to delete it like a regular app. Been through the book marks menu in Safari and Chrome and can’t find it in there.

How to delete a bookmarked web page that I’ve saved to the desktop on an iPhone or iPad?
 
If you do a long press on the icon you should get a popup menu that offers a "Delete bookmark" option.

Just tested on my iPhone.
 
Just tested myself here. Created a bookmark for MacRumors and had no problem deleting in the usual way. But I'm using an iPhone 12 ProMax with some old version of iOS, so that could be the difference.

Are you syncing bookmarks between multiple devices? I stopped doing that - I don't want my computer bookmarks on my phone, and I ran into issues deleting them on other devices. So, wondering if the problem is that you created the bookmark on a different device, and you can only delete it there?
 
I think I am syncing across devices but unsure.
I get thi if I want to delete the NS book marks menu:

Apparently, your second example is a web app and not a bookmark. I'm the author of a web app, and although they can be accessed just like any website, they contain additional resources that allow them to run independently of the web browser, like an app.

My older version of iOS really doesn't make this clear, they appear to just be bookmarks on my home screen when they are actually web apps, so I don't get the same menu you have - guessing this is a feature of newer versions of iOS. But you can tell if it's a web app by going to the app-switcher screen. If it's a web app, it will appear separately instead of in Safari.

But not familiar with "QT"... maybe it's an app store app?
 
Then, my guess would still be that this is related to iCloud bookmark sync across devices. Like I said, I turned it off because it was creating problems for me. I found it confusing, but pretty sure it's not possible to delete a bookmark from only one device if you are using that. Assuming that you still want the bookmark on another device, then I think you would need to turn off iCloud sync on your phone, then delete. But that's not going to be a good solution if you actually want to keep your other bookmarks in sync.
