dell monitor webcam scrambled image

can anyone help? had this issue? where the image comes through but its really scrambled up?

i have a 23" dell monitor and im using the built in webcam, it worked once or twice perfectly, but now it does this...?

if you see the little bit of video coming through at the top, that is actual video i can see myself moving up there..

nobody had this problem? i've had a search around and it seems there is a few more instances of the same thing, but i can't find a solution.
