Dialled caller list not syncing to Mac

As much as people don't want this feature and have problems getting rid of it. I on the other hand want it but its stopped working for me all of a sudden.

Whatever calls i make on my iPhone don't appear on the FaceTime app on the mac and vice versa.

There are times i'll dial 2 numbers and 1 appears.

Is there any way of fixing this?
