General Disappearing Notes

I have an iPhone, iPad and MacBook Pro and I sync my notes through iCloud. For the most part it works well but every once in awhile a note randomly disappears. I checked the settings on each device and all are set to sync notes with iCloud. When I check iTunes, I can't even find a setting that has to do with notes so I assume that is because the iPad & iPhone are set to sync with iCloud. Any ideas what is going on?

if you delete a note from any device it deletes them from all devices, at least thats what happened in my wife's case.
