Why does the occasional picture I take not show up in "Photos," or anywhere else for that matter? Must be the "New and Improved" upgrade a few months ago. . . ?
 
Some apps (WhatsApp, for one) create their own album in Photos and save their photos there.

I've also created a Smart Album named Unsorted, set to show photos that aren't in an album. The condition is set to "Album" and "Is Not" and "Any". When I first created it, it revealed 30-or-so photos I'd forgotten over the years.

Failing that, you might have some weird sync issue.
 
I've not changed anything, I just want ALL photos I take to go into the "Photos" section -- which is how it used to work.
 
the photos you take are probably there - I suggest to scroll down to the bottom of the photos app and customize (e.g. remove the large preview on top, set the order, etc.).

After that go into the library view and use the …-symbol in the top to set the display order to date added to the photo library. That should bring up all photos taken with Apple‘s camera app in the order you expect.

For other apps - as @mblm85 already pointed out - there might be specific albums in which you might find a photo taken from within that app faster than in the library view.

Ultimately update to 18.7.x I would recommend. 🙃
