I noticed this about a week ago when I shutdown my MBA and restarted it. It only shows when first opening Chrome.

Notice the "PH" Favicon on the blank page:

These are the only extensions I have installed:

Some type of virus?

Virus? No.

There's just a favicon from site you visited lingering around and being used by default. If you clear all history, I expect it'll go away.

No, you don't have a virus. It's simply a favicon.

Macs are not immune to malware, but no true viruses exist in the wild that can run on Mac OS X, and there never have been any since it was released over 12 years ago. The only malware in the wild that can affect Mac OS X is a handful of trojans, which can be easily avoided by practicing safe computing (see below). 3rd party antivirus apps are not necessary to keep a Mac malware-free, as long as a user practices safe computing, as described in the following link.

Great news, thanks for the follow-up.

As to the why of it, you'll have to ask the people who wrote the Chrome browser. It's its behavior.
