Until the heart muscle that is being deprived of blood dies, or blood flow to it is restored.

Of course, if too much of the heart muscle dies it stops... then you won't feel any pain at all.

As I understand, there are varying degrees depending on the blockage.

If you have a hard one, you're done in a few seconds to minutes. Lost some relatives that way. A very mild one may go unnoticed. So it depends.
 
Of course he's got large cajones. That's where all the extra blood was stored to keep his heart going.
 
Not a bad effort at all :). Although could have been rather dangerous for the patient at hand.

I don't envy him the job of filling out the longest incident report form he ever :D!

Doctor: No. I can't stop operating on this guy or he's toast.
Nurse: But doctor, we can save you. Not sure about the patient.
