Is Disk Utility most effective when the OS is in Safe Mode?
 
No, once booted, there is no difference for Disk Utility. Safe mode does perform a disk repair while booting, but it does not use Disk Utility for this, strictly speaking.

Why are you asking?
 
Disk Utility is most effective when used on a disk you are not booted to. If you need run it on your boot drive, and don't have a secondary boot drive, booting to Recovery Mode and running DU will get the job done.
 
Some writer in Mac Life recommended Disk Utility be run in Safe Mode.

But obviously, as Hobo has recommended, running it from a secondary boot drive is optimal.

BTW, does the secondary boot drive have to have the exact same OS as the target OS?
