Does an external hard drive gets hot while running games?

My external drive came with a protective leather case and I don't know if is safe to keep it 24/7, is it?

I wouldn't keep in any sort of leather case while using it. Yes usage will increase temps, the more it gets used, the hotter it will get, if you are inhibiting its ability to evacuate the heat, then problems could occur.

Many NAS and DAS units include fans for this very reason

Mine does, even when idling (connected over Thunderbolt. It's a Buffalo HD-PATU3).

I leave it exposed in an air-conditioned room and it's quite warm.
