Just bought my buddies old Ultra first gen that is literally a month old because he upgrade to the new one. It has a screen protector on it which is perfectly installed. I’m debating on whether or not to take it off or leave it on.

Does the Ultra need a screen protector? Is it a good idea to have one on? Anyone else use one?

No you don’t need one unless you are doing crazy things with your watch. The sapphire crystal is scratch resistant. I have no scratches on mine after a year.
 
Personally I don't think it needs one with the Sapphire crystal glass. I have had a Series 6 Gold SS Apple Watch that uses the same material and there are no scratches at all on the screen after three years of use and I have accidentally banged the glass on several surfaces many times without incident.
 
I had 2 steel Apple Watches with sapphire. Both got scratched. I scratched one against the corner of marble kitchen counter. When I took them to Apple for an Apple care plus fix, the guy told me he can send them out but they will probably be declined and that the sapphire is actually a mix and not pure sapphire.
