No the information in your Question is wrong for an upgrade. You don't need all that.

You need last 4 SSN of the Account holder, their ZIPCODE from theri billing address and any PIN or secret password (this is not the web login password )....

It should require the last four of the account holder's social and if the account holder has a billing password on their account, that too.
