I've had this happen to me a couple of times and figured out the pattern - If you're transferring one phone to another with Quick Start, and you go through the process of activating multiple cards in your Wallet (entering CVV, accepting T&Cs, * however many cards you have), you run the risk of tripping over some sort of timeout in the process.
If this happens, you may get a "Data Transfer Cancelled" message, where the only option available is to reset and try again. When you do so, skip each card that is presented during setup - you can activate them later, after setup is complete.
