I have a late 2009 iMac 21.5in 3.06GHz. I've ordered some RAM to upgrade to 8GB as I find it is running slow when I have a few apps open at once. But I've also noticed that it's sluggish when gaming on the current graphics card. Are they relatively easy to upgrade? Could I do this myself or should I ask Apple to do it for me? Has anyone swapped out their graphics card on a late 2009 model?

If your models graphics card is not soldered on logic board its possible
 
But even if you can physically swap the graphics card, that doesn't mean you will have the right Mac firmware plus an OSx driver for it.
You add a graphic card of the same iMac model family A1312 in particular case.Some people have success put 6970M but i think his card is solder in the mobo and only in the 27" model can be done.
