Considering moving from a s2 to maybe the 4 or 5 and was wondering about the ECG.

As I understand it, its passive, in that you have to specifically request a reading. Is this intended to be run at intervals, weekly/monthly etc or as a one off, or just after an alter from the watch?

Struggling to understand how people make best use of this feature.

You can do it whenever you want to, takes 30 seconds. If you think you are having an issue, you can check at any time, so very useful for that. I do it about once a month. As for the best use, probably if and when you are thinking you are having a heart issue, worked very well when my sister noticed an elevated heart rate, she was able to take her information to her doctor.
 
It's a bit of an annoyance for me since unless I've been for a brisk walk at least my resting heart rate will be anything between 40 and 45 (and sometimes down to 36).

The ECG will take a sample of course but will refuse to analyse it unless the heart rate is over 50 :(

Nice to hear someone else in the RHR 40 .. down to mid 30s boat.. Can’t use the LHR warning on the AW.
 
Ok, so it comes down to basically when you don't feel too well, or when you just happen to feel like doing it.

I wasn't sure if it was something that was worth moniotoring monthly for example or is it pretty much fixed from the first reading? Maybe this is now bordering on medical advice :). Do readings change that frequently to warrant doing them regularly?

It really depends on you and your health. It's either going to tell you everything is normal or it's not. Anyone who isn't a doctor isn't going to be able to get more granular than that. I have it because my father died at a fairly young age from a heart attack, and so that has made me paranoid. It's a good thing to have if you are concerned about heart health. If you're not, I would not buy the watch for this feature alone. But, moving from the s2 will bring lots of other improvements too.
