My software is current on both AW8c and iP16P, but the ECG feature on my AW is not working while related data are being collected.
On occasion, ECG runs for about 3 seconds then stops. I have cleaned the watch's sensors.
I do not remember this being easy to use, but do 18.2 and 11.2 contribute to the failure, or is there an issue with the crown?

Clean your crown with an alcohol wipe.
if it still doesn’t work, take it to an Apple Store

And no, 11.2 has nothing to do with it

I did get one reading after several attempts. I would hope the alcohol wipe would not be needed every time as wipes are not often within reach.

Just updated my AW8 to the latest software and ECG doesn't work after multiple shut downs, app reload, and alcohol wipes. Anyone else? Goes 30-29-28 seconds or 30-29 then restarts.

so it recognizes the finger on the crown as otherwise the countdown wouldn't start.
Contact apple and have them run a (remote) diagnostics, most likely part of the troubleshooting will be an un-pair/re-pair. If you decide to do that, set it up as new, do not restore from backup

That’s my next plan, to try the Apple Store. I am under AppleCare plus still. I did a full reset, re-pair yesterday, from scratch not a backup, and I am having the same issue.

You could also call support, they can diagnose remotely, and since you are under AC+ you can do an express replacement, meaning they send you the replacement (a refurb) and you send the original back a few days later. I’ve done that with my Ultra 2 a month or 2 ago with the speaker fainting …
 
It does seem to get more troublesome over time until it reaches the point where it just doesn't reliably run at all. My AW6 did exactly the same thing, not sure if it's age of sensor, but they do seem to fail after a few years
 
ECG is an electrical connection through your body, it's common to have issues with them in the winter due to dry skin. I work with a guy with Xerosis and ECGs in watches will not work for him at all.

See if this helps anyone. Lick your finger and wet your wrist where the watch sits, then lick your finger again and touch the crown. However push the crown toward your wrist from the face so you can put pressure on it without depressing the actual button.
 
My series 10 ECG does not work at all either. They want to send me a refurbished replacement. After spending $750 a year ago on my 3rd watch I am not happy about this at all. What are your thoughts on getting a refurbished replacement . Please advise thanks

Just choose either refurbished or ECG. Easy.

I’ve never seen an Apple refurbished product (I.e., by Apple directly) not look like brand new.

I got both my S7 Ti (battery) as well as my Ultra 2 ( speaker) replaced under AC+ with refurbished models, they look brand new, have new batteries. To the best of my knowledge Apple refurb watches all have new cases, glass, battery seals at the minimum.
You're not saying whether you talked to Apple or some 3rd party, I would certainly trust Apple, but certainly not 3rd parties
