Editable pdf file not working on MacBook Pro

Hi all,

I am trying to fill up an editable pdf form but it won't be saved on Mac. I've been told that the only available version of this form is available as an editable form for Windows only.

Any ideas how to fix this issue so that I can fill it up and save the data into the form.
