I am eligible for an upgrade as I am currently running with a 3gs. Should I pre-order from AT&T website or Apple Website?

Or since I am already on an AT&T plan, can I only pre-order from AT&T?
 
You can pre-order from either Apple or AT&T. I have more faith in Apple handling the transaction/delivery properly that AT&T, so that is where I plan to order.
 
The problem with the last 4 dropped was regardless of who you went through, at&t's "upgrade verification server" was unable to process the traffic going through it.

As an AT&T user you have 4 options

Apple.com
wireless.att.com
The Apple store app on the iphone
& AT&T upgrade app ( coming out tonight) in the app store

Last year the att direct site worked fine for me.

ATT (And Apple for that matter) wouldnt let me purchase from Apple last time because of my company discount (FAN). I had to go through ATT.

Hopefully things have changed.

I wouldn't order from AT&T. I remember people's orders being delayed for both the 3GS and 4. Just go through Apple. It's a much smoother experience.
 
And is it ready to go? As far as sim cards or whatever is needed.

Yes. If it works like last time with the iPhone 4, the new phone will come with a new SIM. You just plug it in to iTunes and activate.

iOS 5 is supposed to support activation over the air (without iTunes), so you may just need to turn on the new phone and click activate.
